Twin Lakes Extended Care is seeking a full-time MDS Coordinator (RN) to join our skilled nursing facility team.
This salaried position offers a $90,000 annual salary, bonus potential up to $7,500 annually based on Quality Measures, and additional earning opportunities tied to facility census growth.
The MDS Coordinator plays a key role in resident assessment, care planning, clinical documentation, and reimbursement processes. We are seeking an organized RN who enjoys working collaboratively with nursing, therapy, and interdisciplinary teams to support quality resident care and regulatory compliance.
Responsibilities
• Coordinate and complete MDS assessments and related documentation
• Ensure timely, accurate, and compliant resident assessments
• Collaborate with nursing, therapy, and interdisciplinary team members
• Support care planning and reimbursement processes
• Monitor assessment schedules and regulatory requirements
• Assist in maintaining accurate clinical documentation
Qualifications
• Active RN license required
• Skilled nursing or long-term care experience required
• MDS experience preferred
• RNs with strong skilled nursing experience and an interest in learning the MDS process are encouraged to apply
• Strong assessment, organization, and documentation skills
