Ayers Health & Rehabilitation Center is seeking an experienced MDS Nurse (RN or LPN) to join our team. This is a high-impact role where your expertise directly influences clinical outcomes, reimbursement accuracy, and overall facility success.

What You’ll Do

  • Complete and submit accurate, timely MDS 3.0 assessments for both skilled (Medicare) and long-term care residents
  • Manage Assessment Reference Dates (ARDs) and MDS scheduling
  • Ensure documentation supports PDPM categories, including wounds and complex clinical conditions
  • Complete Care Area Assessments (CAAs) and support care planning
  • Collaborate with the interdisciplinary team to drive quality outcomes and reimbursement accuracy
  • Review clinical documentation for compliance and optimization opportunities
  • Stay current with Medicare/Medicaid regulations and guidelines
  • Support survey readiness and audit processes

What We’re Looking For (Required)

  • Active RN or LPN license in Florida
  • Proven MDS experience in a skilled nursing or long-term care setting
  • Hands-on experience with skilled (Medicare) MDS assessments
  • Experience with long-term care residents and wound-related coding/documentation
  • Strong knowledge of:
    • MDS 3.0 process
    • RAI Manual & CAAs
    • Medicare/Medicaid reimbursement guidelines (PDPM)
  • Strong attention to detail, organization, and follow-through

Preferred Experience

  • 1–2+ years in a skilled nursing facility (SNF/LTC)
  • Background in wound care, rehab, or case management
  • Experience with PointClickCare (PCC) or similar EMR systems
  • RAC-CT Certification (a plus)
