MDS Coordinator Opportunity:
- MDS Coordinator isresponsible for coordinating and overseeing the Resident AssessmentInstrument process, including Minimum Data Set, 3.0 assessments, Care AreaAssessments, and interdisciplinary care planning in accordance with CMS,CDPH, and federal regulation
- Must coordinate eachassessment with participation of the interdisciplinary team
- collaborates with nursing,therapy, social services, dietary, activities, and business office staffto ensure accurate and timely completion of assessments under OBRA andPDPM
- This role ensures clinicaldocumentation supports MDS coding, reimbursement accuracy, qualitymeasures, and skilled level of care while maintaining compliance andsupporting resident outcomes
MDS Coordinator Qualifications:- Current California RN licenserequired.
- BLS certification
- MDS Coordinator must have aminimum 2 years SNF/LTC/acute experience
- Knowledge of CMS RAI Manual,PDPM, and Quality Measures
- Strong communication,organizational, and EHR skills
