MDS Coordinator (Licensed Nurse)

Role Overview

As our MDS Coordinator, you will lead the comprehensive assessment process with accuracy and care, working closely with our interdisciplinary team to create meaningful, resident-centered care plans. Key Responsibilities

  • Lead the MDS/RAI process, ensuring accuracy, timeliness, and compliance with regulations

Qualifications

  • Active Kentucky RN or LPN license (required)
  • Previous MDS/RAI experience (required)
  • Strong knowledge of PDPM, care planning, and regulatory standards

  • Excellent communication and teamwork abilities
