At Palolo Nursing and Rehabilitation, We are seeking an MDS Coordinator to join our team and support our mission of delivering exceptional care. This role is essential in coordinating and completing the Minimum Data Set (MDS) assessments for all residents in accordance with state and federal regulations. You will work closely with our interdisciplinary team to ensure assessments are accurate, timely, and reflective of each resident’s unique needs.
Salary Range: $100,484.80 ($48.31) and willing to train

As part of our compassionate care team, you will:
Coordinate and complete MDS assessments with accuracy, integrity, and timeliness
Collaborate with nurses, therapists, and other healthcare professionals to gather essential information
Ensure compliance with all state and federal MDS regulations
Review and update individualized care plans based on assessment findings
Communicate assessment outcomes to the care team to support resident-centered planning
Participate in quality improvement initiatives related to MDS and care planning
Maintain organized, up-to-date records of assessments and care plans
Who we are looking for:
Bachelor’s degree in Nursing or related field
Active Registered Nurse (RN) license
Previous MDS experience helpful, but not required — we are willing to train
Understanding of MDS processes and regulations is a plus
Strong attention to detail and organizational skills
Excellent communication and interpersonal abilities
