The Maternal Child Health Nurse manages a case load of antenatal, postpartum, newborn, and pediatric home care patients. The nurse will complete the nursing process; assessment, diagnosis, planning, implementation, and evaluation, for patients in the case load using a family-centered and developmentally appropriate approach. Collaborates with members of the interdisciplinary team, health care providers, and community services to ensure that patient needs are met and utilization of services is effective. Provides clinical oversight of LPNs and LNAs in the home, as necessary.
Responsibilities
Develops a plan of care under the direction of the patient's physician and in collaboration with the patient, their family and/or the primary care givers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate. Initiates referrals for interdisciplinary services and communicates with other disciplines in a timely manner. Provides ongoing assessment of the patient's clinical, functional, and mental status to identify problems and/or progress toward desired goals. Communicates significant changes to the patient's physician and clinical manager as appropriate. Follows the agency policy when necessary mandatory reporting is required. Completes nursing interventions and education relevant to the client. Reinforces caregiver and patient education provided by other health care providers. Accepts responsibility of assignment to perform a specialized procedure and/or see patients in other programs as needed such as, but not limited to, Home Health and Hospice, precepting of students and new staff, etc. Provides clinical oversight of LPNs and LNAs in the home.
