Responsibilities: The Manager of Care Management, under the guidance of the Director of Care Management, provides leadership, coaching and mentoring of the Care Management department professional and administrative staff in order to support and ensure standards of practice for service delivery and organizational performance strategies are consistent with PHC's Mission, Vision and Values. The manager will also provide Care Management services for hospital patients as appropriate to guide care facilitation, coordination, and transitions of care. Qualifications: Education

• Graduate from a recognized, accredited school of nursing Required or
• Bachelor’s Degree in Social Work, Healthcare Administration, or other related field Required

Work Experience

• 5 years If RN: Need combination of of Nursing and Care Management exp in a hospital setting. Experience must include of leadership/Management exp in acute care setting Required or
• 5 years If Social Worker: clinical Social Worker experience, including leadership/management experience in an acute care setting Required
• Required
