Requirements:

• Minimum of 1 year of experience in a senior care or related healthcare setting
• Current CPR certification
• Authorized to work in the US
• Must successfully pass a criminal background check and any additional screenings as required
• Two verifiable professional references
• Must maintain all required certifications, training, and comply with company and state regulations for employment
• Reliable transportation to attend assigned patient visits

Main Responsibilities include but are not limited to:

• Assisting clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) according to the Plan of Care
• Building trusting and confidential relationships with clients and their families
• Maintaining a clean, safe, and comfortable environment for the client
• Providing support with household tasks, personal care, and preparation of nutritious meals
• Assisting with therapy exercises as directed in the Plan of Care
• Monitoring and supporting healthcare needs, including providing medication reminders
• Encouraging clients to maintain or regain independence whenever possible
• Promoting mental stimulation through engagement in activities and conversation
• Accurately and promptly documenting client activities, mood, challenges, and other relevant information
• Reporting any changes in client condition or home environment to the immediate supervisor
• Attending in-service training sessions as required by state and federal regulations
