$35–$40/hr + Mileage Reimbursement

Position Overview

The Clinical Care Coordinator (LVN) serves as a liaison between patients, families, referral sources, and the hospice/palliative care team. This field-based role provides education, conducts nursing assessments, coordinates admissions, and helps ensure patients receive timely, compassionate care.

Responsibilities

Conduct nursing assessments and admissions
Review consents along with other documents and assists with election of hospice benefit.
Educate patients and families about hospice and palliative care
Coordinate care transitions from hospitals and facilities
Collaborate with interdisciplinary teams
Complete documentation using EMR systems

Qualifications

Current California LVN License
1+ Year Nursing Experience, hospice experience highly preferred
Strong Communication Skills
Reliable Transportation & Valid Driver’s License
Bilingual Spanish Preferred
