Summary:
Work Mode: Remote-NC, Region 2, EST zone
Duration: 5 months, extension and/or conversion possible but not guaranteed
Must be able to travel and make face-to-face visits to members in Region 2 counties, with potential for > 50% travel
Shift: 8-hour day, 40-hour work week, 8am-5pm EST Mon – Friday
Responsibilities:
Develop, assess, and coordinate holistic care management activities to support quality, cost-effective healthcare outcomes
Create or support personalized service plans for long-term care members
Educate members, families, and caregivers about available services and benefits
Work collaboratively with a team including utilization management, quality, transition of care, care managers, coordinators, and navigators
Requirements:
Bachelor's degree required
Minimum two years of experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management
Prior experience in social work, geriatrics, gerontology, pediatrics, or human services
Must have an unrestricted driver's and practice license
Preferred Skills:
RN or LCSW licensure preferred
Prior managed care experience
Strong care management experience
Effective communication skills
Proficient computer skills
