About the Role:
As a Licensed Practical Nurse (LPN) in our Virtual Care Center, you will serve as the key point of contact for patients and staff by providing exceptional customer service and high-quality patient care. You will be responsible for managing high-volume triage calls, assessing urgent patient needs, supporting medication prior authorization, and supporting various clinical and administrative functions that ensure efficient and compassionate care delivery.

How You’ll Drive Our Mission Forward:

Triage incoming calls to determine urgency and coordinate timely medical response

Accurately document patient complaints, symptoms, and actions taken in the electronic medical record (EMR)

Support risk management by maintaining precise clinical documentation

Communicate directly with providers about patient concerns and clinical status

Provide education to patients and families, ensuring understanding of care plans and instructions

Support the overall flow of clinic operations through scheduling, medication refills, patient messaging, and medication prior authorization

What Makes You a Great Match:

High school diploma required

Active LPN license issued by the Board of Nursing

Basic Life Support (BLS) certification required

Advanced Cardiovascular Life Support (ACLS) certification optional

Minimum of 2 years of LPN experience, or 1 year of cardiology LPN experience

Excellent communication and customer service skills

Familiarity with electronic medical record systems and clinical documentation

1 year of experience with medication prior authorization through Cover My Meds or Sure Scripts
