Job Summary
The LPN/RN MDS Coordinator plays a crucial role in ensuring compliance with federal and state regulations regarding patient care documentation and assessment. This position involves coordinating the completion of the Minimum Data Set (MDS) assessments for residents in skilled nursing facilities, ensuring that all documentation meets the required standards. The MDS Coordinator will work closely with interdisciplinary teams to facilitate effective case management, utilization review, and quality assurance processes.

Duties

  • Oversee the completion of MDS assessments and ensure timely submission to Medicare and Medicaid.
  • Conduct thorough documentation reviews to ensure compliance with HIPAA regulations and facility standards.
  • Collaborate with nursing staff and other healthcare professionals to gather necessary medical documentation for accurate assessments.
  • Participate in utilization management and review processes to optimize patient care and resource allocation.
  • Monitor patient care plans, particularly for those in inpatient settings or requiring hospice care, ensuring they meet critical care standards.
  • Provide training and support to staff on MDS processes, documentation requirements, and best practices in case management.
  • Analyze data from MDS submissions to identify trends and areas for improvement in patient care.

Requirements

  • Proven experience in MDS coordination or related roles within healthcare settings.
  • Strong knowledge of utilization review, case management, and utilization management principles.
  • Familiarity with inpatient care protocols, hospice care standards, and critical care practices.
  • Excellent organizational skills with a keen eye for detail in documentation review.
  • Ability to work collaboratively within interdisciplinary teams while maintaining a focus on patient-centered care.
  • Understanding of HIPAA regulations and commitment to maintaining patient confidentiality.
  • Strong communication skills, both written and verbal, to effectively convey information across various levels of the organization.

Summary of Qualifications

  • A graduate of an accredited school of Nursing and currently licensed as a Nurse in the State is required.
  • Previous work experience in a long-term care setting. Supervisory experience and organizational skills in a long term care setting are preferred.
  • Is listed in good standing as confirmed on the State Department of Professional Credentialing.
  • Possesses excellent technical assessment and documentation skills, and leadership qualities.
  • Possesses good communication skills and is patient and self-disciplined.
  • Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
  • Knowledge of MDS rules and regulations, including item coding, RAPS, care planning, electronic submission, scheduling, and working knowledge of the relationship between PPS and MDS, etc.
