This role is an LPN who supports our Quality and Data Analytics team. Primary duties to include:
• Entering and preparing AWV-related orders in advance of patient visits • Conducting outreach calls to patients • Reviewing documentation for completeness for providers • Coordinating with practices for scheduling patients
This role works with all TMG practice providers, managers, and directors to ensure patient care gaps are addressed in a timely manner.
Required Knowledge & Skills
Education: Associate Degree or 2 years of college coursework
Experience: One year of related experience. Requires a working knowledge of standard practices and procedures.
Licenses and Certifications Georgia Licensed Practical Nurse or Multistate Nursing License
Qualifications Completion of a formal education program in practical nursing Current state LPN licensure Prior outpatient quality or population health experience strongly preferred Experience with EMRs, data abstraction, patient outreach, or value-based care initiatives preferred.
