Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:

Job Title: Licensed Practical Nurse (LPN) Primary Care
Reports to: Primary Care Nurse Supervisor
Areas of Responsibility: Function as LPN in the provision of health care and educational/screening programs.
Major Duties:
Work within the LPN's Scope of Practice performing skills including, but not limited to the following:
Patient assessment and monitoring, including preventative screenings
Prepare patients for examinations and explaining procedures
Administer medication as ordered
Administering immunizations
Assist with minor procedures
Wound care and dressing change
Data collection and reporting
Routine lad and sample collection including phlebotomy
Triage and assist in emergency situations as needed, according to the provider's written, verbal, or electronic orders
Provide emotional support and counseling to patients, caregivers and families as needed
Assist with the external referral process, assist with medication refills, and respond to patient inquiries with timely follow-up.
Assist with coordinating patient appointments and follow up care as needed.
Care-manage chronic diseases in patients by identifying barriers to care and offering information related to specific diseases and initiating referrals for needed in-house services (Pharmacist, Diabetes Educator, Dietician, Social Worker etc.) to improve patient outcomes.
Act as a patient advocate and practice effective patient communication.
Assist patients to complete appropriate applications for eligibility of patient assistance medication programs; coordinates and monitors the need for reordering medications from patient assistance medication programs.
Provide Health Education to patients and families about their conditions, medications, at-home self-care plans and health promotion activities.
Work cooperatively with all CAPWN Health Center staff, other agencies and programs, and with all program staff in meeting agency/program goals; Become knowledgeable of community and CAPWN resources.
Ensure clinical areas and equipment are clean and sterile.
Ensure compliance with healthcare regulations and standards
Assist in the management of medical supplies and inventory
Practice cultural competence with the culturally diverse population this clinic serves.
Effectively communicates while interacting with patients, families and staff and recognizing the complexity in language interpretation.
Maintain a positive and professional attitude, striving to openly communicate with co-workers and patients, breaking down communication barriers when necessary.
Participate in Quality Assurance and Improvement initiatives to ensure the highest standards of patient care are met.
Maintain confidentiality of records and communications following CAPWN policy and HIPAA privacy and security policies and procedures.
Complete all documentation thoroughly and in a timely manner to ensure patient records are up-to-date and accurate.
Utilize the Electronic Medical Record (EMR) to assist in the delivery of quality care and track patient care.
Attend staff meetings, training, and workshops or classes as requested/required by Program Managers, Directors.
Maintain current BLS (Basic Life Support)
Maintain continuing education requirements.
