This position is located at the Department of Mental Health, Waccamaw Center for Mental Health, 164 Waccamaw Medical Park Drive, Conway, SC 29526

Under direct supervision, plans, implements and provides direct nursing care to all patients of the Conway office for WCMH. In collaboration with the treatment team, will evaluate the effectiveness of care/intervention and discharge planning. This position will require walking and assisting patients through the clinic. The position will include providing injections to patients, taking vitals and providing information to physicians, storing medications in accordance with SC Board of Pharmacy. Works closely with staff, patients, nurses and business/front office to enhance quality care. This position is considered essential and therefore the employee may be asked to report to work during emergencies or center closure.

Responsibilities for the LPN Non-Institutional Include:

  • Provide direct patient care - injections, weight, blood pressure, height and pluses rate by physicians and nurses.
  • Assist with prior authorization requests as appropriate and seeks physicians assistance as needed.
  • Perform urine drug screens and other necessary tasks as requested by immediate supervisor and Clinic Director.
  • Maintain patient care hours per productivity schedule.
  • Make observations, administer medication (oral and injections,) educate patient and families through teaching and rehabilitative measures, and utilize other appropriate resources as needed.
  • Coordinate needed community support and work closely with Community Support Staff members.
  • Complete all required paperwork for medication assistance, which includes logging in medications that arrive at the clinic, reviewing the clients medication order and contacting clients for medication pick up. Must submit clinical service notes within a 24-hour period in accordance with Local Collaborative Documentation Standards.
  • Maintain documentation of charts and other records in accordance with State, Federal, Local, OMH, WCMH, Quality Assurance, Corporate Compliance and CARF Standards.
  • Applicant needs to have experience working in a mental health setting with knowledge of practical nursing procedures and the ability to make accurate and technical and physical observations of patients.
  • Knowledge is needed of the services provided in a comprehensive mental health programs service children, youth and adults.
  • Serves as a backup when needed for front desk check in and check out patients.
  • Support with stocking medications, triage patients when needed, provide coverage for patients when other nurses are out of clinic.
  • Follow up calls to be made to physicians and nurses when completing forms for labs and assist with referrals from physicians as needed.
  • Consultation to nursing and medical team members when required.

Minimum and Additional Requirements

  • Graduation from an accredited school of nursing.
  • Must have licensure as LPN through the SC Board of Nursing, SC LLR.

Additional Requirements:

  • Ability to lift up to forty (40) pounds; ability to see, hear at normal volumes, stand, walk, reach, bend, stoop, pull and climb at least two (2) flights of stairs.
  • Ability to tolerate noise level and circulate in patient areas.
  • Must be able to work with repetitive patient behaviors.

Preferred Qualifications

  • Bilingual abilities in English and Spanish (or another language) are a plus.
  • Candidates currently living in the catchment area are preferred.

Post hire, Employee Must:

  • Be able to provide a 10-year driving record.
  • Be able to use a state vehicle for work related purposes.
  • Use clinical judgement while working independently and be self-directed.
  • Communicate effectively.
  • Have the capacity to establish and maintain effective working relationships with patients and families.
  • Participate in on-going in-service training and other educational opportunities in order to maintain and acquire necessary skills to perform all duties.
