- # of Beds/Residents: 80 beds / Current 51 Census. Has 4 main hallways, all ages, male and female. 1 locked memory care unit with 14 beds
- EMR: PCC
- Ratios: 1:25
- Required Certifications: BLS
- Common diagnosis / Types of patients: Skilled Care/LTC/Multiple Comorbidities
PTSD
- Job Duties: Assess & evaluate residents, Administer meds/treatments, veni-punture blood draws. Communicate condition changes, Lead care coordination. RNs & LPNs given access to HUCU system for medical orders and communications
- Experience or 'Must have' skills: Must have 1 year of LTC/Skilled. Must have some memory care/PTSD experience. Must have experience with venipuncture blood draws
- Are there IV's or titratable drips?: Accept IV drips: antibiotics (rare)
- Area there Tele monitors?: Nurses manage telehealth visits
- Are RN's required to attend team conferences for acute inpatient rehab? None
- DON/Charge Nurse/House Supervisor: We have a DON and ADON. No Charge nurse or House Supervisor. DON/ADON/MDS cover call. Leadership cover call 24/7.
- Physician availability after hours: We have an on-call physician but we are a 24 hour facility.
- Interdisciplinary Team: PT/OT/SLP, Pulmonary or Cardiac Rehab, Social Workers, Dieticians
- Is RT on-site 24/7: RN/LPN to manager breathing treatments and respiratory equipment, no RT onsite
- Pharmacy on-site?: No pharmacy on site. Nexus is our dispensing system
- Shifts & Scheduling: 3-on/3-off 12 hour shifts. Each week shifts by 1 day due to the 3-on/3-off. Extra shifts available- prefer someone who will pick up shifts. Leadership ask that all time off requests are made prior to start of assignment and that travelers are flexible and help to fill gaps in permanent scheduling
- Weekend Requirements: 3 weekends on and then 3 weekend offs
- Holiday Requirements: May be asked to go into the Holiday Rotation (depends on how it all falls)
- Scrub Color/Dress Code: Scrubs of any color so long as they are clean and in good repair
- Unit/Department Specific Orientation: 1 hour new-traveler onboarding with Sidreis and 1 hour orientation with ADON. 2 shifts with a preceptor on the floor
- Additional Notes: Remote rural location, An hour to a larger city that would have Walmart, fast food, etc. Winter is it very cold and snowy, gets dark at 4 PM and doesn't get light until 9 AM.
Team atmosphere and looking for someone that is going to jump in wherever is needed
