What you will do as a Hospice LPN:
- Provide compassionate, direct patient care under the supervision of a Registered Nurse following each patient's individualized plan of care.
- Deliver treatments, monitor patient conditions, and report changes promptly to the RN Care Manager.
- Assist with pain and symptom management to promote comfort and quality of life.
- Educate patients and family caregivers about diagnoses, medications, treatments, and the plan of care.
- Accurately document all patient care, assessments, and interventions in the electronic medical record.
- Collaborate with physicians and the interdisciplinary team to coordinate exceptional hospice care.
- Safely prepare and utilize medical equipment and supplies while following infection control and safety standards.
- Participate in interdisciplinary team meetings, staff meetings, continuing education, and quality improvement initiatives.
- Support Good Samaritan's mission by providing compassionate, respectful, and patient-centered care in every interaction.
- Participate in the nursing holiday rotation.
What we are looking for:
- Graduate of an accredited Practical Nursing program.
- Current Virginia Licensed Practical Nurse (LPN) license.
- Minimum of three (3) years of nursing experience; hospice, home health, community health, or medical-surgical experience preferred.
- Experience with electronic medical records (EMR).
- Current driver's license, reliable transportation, and acceptable driving record.
- Current CPR certification (or ability to obtain per organizational requirements)
Schedule & Benefits:
Schedule: M-F 8:30am -5:00pm with rotating weekend and holiday on-call backup responsibilities
Location: Serving patients throughout the Greater Roanoke Area including homes, facilities, and hospitals
