Were looking for a compassionate and dedicated Home Health LPN to become part of our expanding team serving the Tri-Cities area. This role is ideal for nurses who enjoy using their clinical skills independently while building meaningful relationships with patients. Youll provide skilled nursing care in the home, including wound care, catheter care, ostomy care, disease management, patient education, and ongoing patient assessment.
Home Health experience is preferred but not required. We welcome LPNs with strong clinical experience from skilled nursing facilities, rehab hospitals, long-term care, subacute rehab, acute care hospital settings, LTAC, assisted living treatment teams, wound care clinics, dialysis, and other skilled nursing environments.

Brief Overview of Responsibilities: As a Home Health LPN, youll provide skilled nursing care to patients in their homes including wound care, catheter care, ostomy care, disease management, patient education, and ongoing monitoring. Youll work closely with the RN, physician, patients, and families to ensure high-quality care while helping patients maintain their independence and remain safely at home.
