Contract Length: 13 Weeks (05/12/2026 – 08/15/2026)
Schedule: Full-Time, 5x8-hour evening shifts
Shift Time: Evenings (e.g., 3:00 PM – 11:30 PM)
Hours: 40 hrs/week
Pay: $1,063 – $1,204/week
Requirements: Every Other Weekend (EOW) + Holidays required
Provide skilled nursing care to adult/geriatric residents under RN supervision
Assess, monitor, and report physical/psychosocial status changes
Administer treatments/medications and support ADLs per care plan
Contribute to and update individualized care plans with interdisciplinary team
Maintain accurate documentation and ensure patient safety/compliance
Active NY LPN Licensure
BLS Certification (AHA or RQI required)
Minimum 1 year LPN experience (preferably in LTC setting)
Ability to care for adult and geriatric populations
High School Diploma or GED (plus accredited LPN program)
Willingness to work in environment with potential exposure to blood/body fluids.
