Join us as we deliver hope and healing to the patients and families who need it most.

    ROLE SUMMARY:

The Patient Access & Care Team LPN (PACT LPN) plays a critical role in supporting patient care by managing electronic communications within the electronic health record (EHR). This role is responsible for reviewing, prioritizing, and responding to patient messages, refill requests, and other provider-directed communications in a timely and clinically appropriate manner.

Working in close collaboration with providers, the triage RN, and the broader care team, the LPN helps ensure safe, efficient, and patient-centered communication and follow-through. This position requires strong clinical judgment within LPN scope, excellent written communication skills, attention to detail, and the ability to manage a high volume of tasks in a fast-paced, team-based environment.

The ideal candidate demonstrates professionalism, flexibility, and a positive, team-oriented mindset, and is energized by working collaboratively to build and support a strong, mission-driven team culture. As this is an evolving role, the LPN must be adaptable and open to ongoing workflow development and continuous improvement.

LPNs in this role are expected to practice in accordance with the Ohio Nurse Practice Act and within their licensed scope.
