The Staff Pad has partnered with a premier hospital in Manchester, TN, to hire experienced an LPN Case Manager for their ER department.<\/span><\/span>
<\/p>

In this role, you'll coordinate patient care from admission through discharge, ensuring quality outcomes, seamless transitions, and an exceptional patient experience.<\/span><\/span>
<\/p>

The Case Manager collaborates closely with physicians, nurses, and care teams to ensure appropriate level of care, efficient throughput, and safe patient transitions.<\/span><\/span>

What You’ll Do<\/span><\/span>
<\/p>

Responsibilities <\/span><\/span>
<\/p>

  • Assess patient needs and develop individualized care plans<\/span><\/span>
    <\/li>
  • Conduct utilization reviews to ensure medical necessity and appropriate level -of -care decisions<\/span><\/span>
    <\/li>
  • Coordinate admissions, transfers, discharge planning, and community resource referrals<\/span><\/span>
    <\/li>
  • Collaborate with physicians, nurses, and interdisciplinary teams to ensure seamless care<\/span><\/span>
    <\/li>
  • Identify and resolve barriers to care transitions and patient flow<\/span><\/span>
    <\/li>
  • Educate patients and families on treatment plans and follow -up care<\/span><\/span>
    <\/li>
  • Monitor patient outcomes and identify opportunities for improvement<\/span><\/span>
    <\/li>
  • Ensure compliance with hospital policies, regulatory standards, documentation and payer requirements<\/span><\/span>
    <\/li>
  • Advocate for patients throughout their healthcare journey<\/span><\/span>
    <\/li><\/ul>

    Qualifications<\/span><\/span>
    <\/p>

    • Graduate of an accredited Practical Nursing (LPN) program<\/span><\/span>
      <\/li>
    • Current LPN license (Tennessee or compact state)<\/span><\/span>
      <\/li>
    • BLS certification required, ACLS preferred<\/span><\/span>
      <\/li>
    • Minimum 1 -2 years of LPN experience, preferably in an ER/acute care or Med -Surg setting<\/span><\/span>
      <\/li>
    • Previous case management or utilization review experience preferred<\/span><\/span>
      <\/li>
    • Strong clinical judgement, communication, critical -thinking, and organizational skills<\/span><\/span>
      <\/li>
    • Knowledge of care coordination, utilization review, and discharge planning<\/span><\/span>
      <\/li>
    • Excellent teamwork abilities and able to thrive in a fast -paced high -pressure environment <\/span><\/span>
