LICENSED PRACTICAL NURSE JOB DESCRIPTION Supervisor: Director of Nursing Shift: 11pm-7am Start Date: 8/28/2026 Status: Part Time: Every Friday Benefits: Paid Time Off, 403B, Credit Union, Paid Orientation, Employee Referral Program, Opportunites for Advancement. Qualification: Current licensure as a Licensed Practical Nurse by the New Jerssey Boad of Nursing. Previous experience in a geriatric long-term care facility preferred. Ability to read, write speak and understand the English language required. Purpose: The primary purpose of this position is to provide direct nursing care to the residents and to assist in the supervison of the day-to-day activitied performed by other nurses and nursing assistants. Such supervision must be in accordance with current federal, state and local standards, guidlines and regulations that govern our facility and as may be required by the Director of Nursing to ensure that the highest degree of quality care is maintained at all times. Functions: Ability to follow established standards of nursing practices, facility policies and procedures. Ability to assit in obtaining reports from off-going shift nurse and to assist with recording of sufficient information to implement appropriate follow-up actions as necessary. Ability to assist in ensuring that all nursing assistants comply with the current rules, regulations and guidlines governing the long-term care facility. Ability to assist to interpret existing policies and procedures to nursing assistants, residents, families/sponsors and physicians. Ability to assist in directing the day-to-day functions of the nursing assistants in accordance with current rules, regulations and guidelines that govern the long-term care facility. Ability to assist to provide reports tot he on-coming shift nurse which includes sufficient information for follow-up actions as necessary. Ability to conduct rounds at the beginning, during and end of the shift as part of the shift report to identify and resident problems or concerns. Ability to identify safety hazards, equipment failure or buidling problems and to initiate and/or assist in initiating appropriate measures, including notification to appropriate administrative staff. Ability to identify reisent problems and emergency situations and to initiate and/or to assist in initiating appropriate measures, including notification to appropriate administrative staff. Abilkity to attend in-service programs as assigned and required. Ability to participate in the employee evaluation process by providing information on the clinical performance of nursing assistants to the Director of Nursing. Ability to administer medictions and treatments in accordance with physician's prder and accurately document all care provided. Ability to prepare and administer medications, enteral nutrition and treatments as ordered. Ability to supervise the application and removal of orthotic devices and postural supports. Ability to administer and evaluate the effectiveness of PRN medications and treatments appropriately and to report changes to the Director of Nursing and Assistant Director of Nursing. Ability to notify thephysician, when necessary, of changes in the resident's condition, incident/accidents, significant weight changes, medication errors, medications not available, abnormal laboratory/x-ray results and any other unusual observations. Ability to assess for injuries resulting from an incident/accident involving residents and employees and to initiate and/or to assist in initiating appropriate care as well as proper notification of physician, family/sponsor and supervisor. This includes completing incident/accident reports. Ability to perform charting duties as required in accordance with established charting and documentation policies and procedures. Ability to assist to perform and document the comprehensive assessment on each resident. Ability to participate in the development of an accurate resident plan of care based on the comprehensive assessment. Ability to document resident care provided and the resident's esponse to the individualized plan of care. Ability to assist in evaluate, review and revision of the resident care plan, as necessary and required by regulations. Ability to listen to resident, family/sponsor or physician's concerns and to assist in initiating corrective actions and/or report concerns to the Direcgtor fo Nursing. Ability to order medications following the guidelines established by the provider pharmacy. Ability to count controlled drugs and syringes, check emergency kit and suction equipment. Ability to report to, and receive from, nursing assistants any changes in resident condition. Ability to explain all pertinent resident care tot he nurses and nursing assistants. Ability to identify the appropriate use of restraint devices following OBRA guidelines requiring physician orders, proper documentation and follow-up. Ability to check meal trays to ensure proper diet is serviced and residents are assisted as necessary (7:00am-3:00pm and 3:00pm-11:00pm shifts). Ability to document on the resident medical record on all unstable or recently admitted residents. All entries will include signature and date. Ability to report to work as scheduled and consistently demonstrate dependability and punctuality. Ability to conduct self and carry out assigned duties in a professional and cooperative manner. Ability to consistently work cooperatively with residents, nursing staff, physicians, families/sponsors, consultant personnel, community agencies and ancillary service providers. Ability to participate in surveys (inspections) made by authorized government agencies. Ability to observe all facility policies and procedures, including universal precautions, infection control, fire prevention and safety. Abilkity to follow Resident's Rights policies at all times. This includes the ability to maintain the confidentially or resident care and financial information, knocking before entering. the resident's room, providing all nursing care in privacy and treating all residents fairly and with kindness, dignity and respect. Ability to assume accountability for data contained in the employee's handbook. Ability to perform other related duties as directed by the Director of Nursing. Ability to assume chage of the Nursing Unit. Physical and Sensory Requirements (with or without the aide of mechanical device): Must be able to move intermittently throughout the workday. Must be able to cope with the mental and emotional stress of the position. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must function independently, have flexibility, personal integrity and the ability to work effectively with residents, personnel and support agencies. Must be in good general health and demonstrate emotional stability. Must be able to relate to and work with the ill, disables, elderly, emotionally upset, and at times, hostile people within the facility. Must be able to list and move residents, equipment, supplies, etc., inexcess of fifty (50) pounds. Must be able to assist in the evacuation of residents. Requirements:
