**Estimated Credentialing Timeframe: 30-days, will consider emergency privileging

Practice Setting: Hospital Based Rounding

Schedule (Shifts and Hours): 24 hour call shifts

Call Ratio: needing 10-14 days call coverage per month

Number of Operating Rooms: 20

Documentation System/EMR: EPIC

Description of Ancillary Staff: Support staff includes 3 PA's

Patient Age Demographic (Pediatric, Adolescent, Adult, Geriatric): Adult

REQUIRED PROCEDURES:

Coronary Artery Bypass Surgery Procedures Required:

CABG

Arterial Revascularization (using internal mammary arteries, radial artery and gastroepiploic artery)

Redo Coronary Revascularization (With or Without Heart Valve Surgery)

Pericardial Window

Heart Valve Repair/Replacement surgery

Mitral Valve Repair
