Coverage Location: St. Joseph Warren Hospital, Warren, Ohio
Practice Setting: L&D, OB/GYN
Reason for Coverage: We do not have enough employeed physicians and midwives to cover the laborist model.
Dates of Coverage: Starting as soon as possible until we hire more providers.
Schedule: Physicians 7a-7p, Midwives 7p-7a
Call Schedule: Physician call for midwife 7p-7a
Average Number of Scheduled Patients Per Day: N/A - not outpatient
Number of Physicians in Practice: 1 full time laborist physician and 1 full time laborist midwife
Will Other OB/GYNs Provide Coverage During Locums Coverage Dates: Nicole Carlson, MD, Chardina Murray Mangino, CNM

Required Obstetrics Procedures:
Low Risk Pregnancy (i.e. History of Twins, Toxemia, Pyelonephritis, etc., High Risk Pregnancy (i.e. with Toxemia, Diabetes, Class II or Higher Cardiacs, Chronic Lung Problems, Chronic Renal Disease, etc.)
Labor and Delivery Required Procedures:
Routine Delivery Vaginal, Low Forceps, Decision-Making for C-Section, Complicated Delivery C-Section, Breech, Multiple Births, Abruptions, etc., VBAC s, Repair of 3rd & 4th Degree Lacerations
Gynecology Required Procedures:
Laparoscopic Diagnostic/Minor Only-Tubal Ligation, LOA s, Chromal Tubation, Surgical Gynecology Open
Estimated Credentialing Timeframe: 120 days
Facility Reimburse for the Following Costs: Flight, Hotel, Rental Car, Mileage Billed at the IRS Standard Rate
