Orientation $125-137.50 - half the hourly/callback rate, kicks in after 8 hours. Per hour of orientation or training beyond 8 hours
Hourly $250-275 - Per hour, not OT
Holiday $375-412.50 - Must be 1.5x the hourly rate. Per holiday hour, no OT
Nightly Call $600-$700 - Per night call to hold the pager, to include 2.5 hours gratis
Holiday Nightly Call - $900-1050 - "Per night call to hold the pager, to include 2.5 hours gratis
On Call (Daily) $1,500-$1,700 - Per 24 hours to include 4 hours gratis"
24 Hour Holiday Call Rate - $2250-2550 - Per 24 hours to include 4 hours gratis"
Call Back $250-$275 Per hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
Holiday Call Back $375-412.50 Per holiday hour of patient contact hours, after gratis hours are completed, gratis prorate to MSA terms.
