We are seeking a dependable Licensed Practical Nurse (LPN) for a local contract assignment in a skilled nursing facility. The LPN will provide direct patient care, administer medications, and support the interdisciplinary care team to ensure high-quality resident outcomes.
Key Responsibilities
Provide direct nursing care under RN supervision
Administer medications and treatments as prescribed
Monitor and document patient conditions and changes
Assist with admissions, discharges, and care planning
Support resident daily care needs as required
Maintain accurate and timely clinical documentation
Ensure compliance with facility and regulatory standards
Requirements
Active Licensed Practical Nurse (LPN) license
Current BLS certification (if required)
Strong clinical and communication skills
Experience in long-term care preferred
Ability to work in a structured healthcare environment
Reliable transportation for local contract assignment
