This role focuses on coordinating patient care after hospital discharge, reducing readmissions, and improving patient outcomes through proactive outreach and care coordination.Requirements:
- Active RN License
- BLS Certification
- Epic experience required
- Experience in Case Management, Care Coordination, or Transitional Care
- Strong knowledge of Medicare guidelines, HIPAA, and discharge planningKey Responsibilities:
- Conduct post-discharge outreach within required timeframes
- Complete medication reconciliation and discharge assessments
- Coordinate follow-up appointments with PCPs and specialists
- Identify barriers to care and assess social determinants of health
- Monitor high-risk patients and escalate clinical concerns
- Collaborate with providers, social workers, and community resources
- Maintain accurate TCM documentation in EpicPreferred Experience:
- Transitional Care Management (TCM)
- Annual Wellness Visits (AWV)
- Care Coordination and Population Health
