Responsibilities:

  • Manage inpatient discharge planning for medical-surgical and critical care patients.
  • Collaborate with multidisciplinary teams including case management and social work.
  • Facilitate patient transitions from hospital to next care setting.
  • Hit the ground running with about 3 days of orientation.
  • Support staffing needs with potential coverage of 1 to 2 units (20 beds per unit) during crisis periods.
  • Cover every 4th weekend and one holiday per year per schedule.
