Position Details

Position: Licensed Vocational Nurse (LVN) - PRN (As Needed)

Location: Tice Valley Post Acute - Walnut Creek, CA

Pay Rate: $35.00 per hour

Schedule: PRN with flexible scheduling based on facility needs

Key Responsibilities

Clinical Care

  • Provide skilled nursing care, including medication administration, treatments, wound care, and monitoring resident conditions.

  • Deliver compassionate, resident-centered care that promotes comfort, dignity, and safety.

Assessment & Monitoring

  • Observe, assess, and document changes in residents' conditions.

  • Report significant findings promptly to the RN Supervisor and attending physicians.

Care Planning

  • Assist with implementing individualized care plans.

  • Support residents in achieving rehabilitation goals while maintaining the highest quality of care.

Team Collaboration

  • Supervise and support Certified Nursing Assistants (CNAs).

  • Work closely with physicians, therapists, and the interdisciplinary care team to ensure continuity of care.

Documentation & Compliance

  • Maintain accurate and timely clinical documentation.

  • Follow all federal, state, and facility policies, procedures, and regulatory requirements.

Qualifications

  • Current and active California LVN license required

  • Current BLS/CPR certification required

  • Skilled nursing or post-acute care experience preferred

  • Strong clinical assessment, communication, and organizational skills

  • Compassionate, dependable, and team-oriented with a commitment to exceptional resident care
