Mobile Health Team Inc. is seeking an experienced Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)- Home Health for a 13-week travel assignment in Pittsfield, MA.


Contract Details
  • Days 8:00 AM - 4:30 PM 8hr Days 40hr gt
  • Start Date: 8/17/2026
  • Contract: 13 Weeks
  • Location: Pittsfield, MA
  • Hourly Pay: $ 24.00
  • Non-taxed Stipend: $ 1204.00
  • Weekly Total Pay: $ 1924.00
  • Weekend Requirements: None/Holiday coverage if required
  • Traveler Policy: CLIENT PREFERS 45 MIN OR LESS COMMUTE FROM TOR OFFICE (165 Tor Ct, Pittsfield, MA 01201)
  • Non-Billable Orientation: 8hrs

** Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION. Facility will NOT approve scheduling requests at/after time of offer/at interview.

The Community Health LPN provides and documents nursing care of the sick and injured at home.

  • The LPN intervenes to promote, maintain or restore health to prevent illness and to effect rehabilitation
  • The LPN collaborates with other healthcare providers, professionals and community representatives to coordinate care at home
  • The LPN evaluates the individual’s and families’ response to interventions.
  • Ability to function independently under general guidance of the Director and follows the guidelines and protocols and/or policies and protocols specific to each unit
  • Duties require independent action and judgment
  • Utilizes time management, communication, planning, delegating, and problem solving in providing care.

Key Responsibilities:

  • Observe patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action.
  • Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, or respiration.
  • Administer prescribed medications or start intravenous fluids, noting times and amounts on patients' charts.

Compensation:

  • Weekly Pay (hourly) + Non-taxed Stipend (if eligible)

Requirements

Minimum Requirements:

  • Must have One year acute nursing care, hospital based preferred.
  • Must possess and demonstrate critical thinking skills. Must maintain emotional stability in emergency settings.
  • Experience in home health is required
  • Certified Home Health Required Including Proficiency with OASIS in highly preferred
  • Must have Massachusetts LPN license
  • Infusion Therapy Experience is preferred
  • Demonstrated knowledge of the principles and practices of nursing.
  • Knowledge of and ability to apply community health principles and practice.
  • Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.
  • Experience with self-scheduling of caseload preferred.
  • History of flexibility in assuming varied assignments.
  • Current Driver License (and insurance) [Required]
  • In order to effectively provide home care, they must have a working knowledge of home care reimbursement and regulatory guidelines.
  • Traveler candidate must have their own vehicle. This is a firm requirement.
  • Traveler perm address must be included in profile
  • Ability to exercise independent judgement.
  • Demonstrated interpersonal skills.
  • Ability to maintain confidential information
  • Ability to develop and maintain records and communicate clinical data in verbal and written form
  • Willing to be flexible regarding work hours.
  • Ability to drive an automobile in inclement weather.
  • Must be willing to accept “on call” responsibilities.

  • Conforms with Patient’s Rights Policy.
  • History of dependability in meeting demands of committed work schedule.
  • Must have and maintain current Basic Life Support (BLS) certification during employment
  • Excellent communication skills. Ability to work independently.
