What You'll Do

• Provide direct patient home care visits, observe, perform, and report preventative, treatment, and rehabilitative nursing procedures while implementing the plan of care
• Observe and report on the home environment and make recommendations that will enhance patient care, promote safety, and prevent adverse effects
• Work with patients, families, and healthcare team members to coordinate services, promote quality of care, and support positive patient outcomes
• Take verbal orders from Health Care Providers as necessary and within the LVN scope of practice in a timely manner and document appropriately
• Inform Health Care Providers of changes in patient condition in accordance with provider orders and document communications accordingly
• Provide preventive, rehabilitative, therapeutic, comfort, and pain interventions as ordered
• Provide timely instruction and education to patients, caregivers, and staff

Qualifications

• Graduation from an accredited nursing program
• Current state license as a Licensed Vocational/Practical Nurse and CPR certification
• At least one year of clinical practical nursing experience (Home Health experience preferred)
• Must have a vehicle, valid driver’s license, and automobile insurance
• Ability to make home visits and provide skilled nursing assessments and care in the home
