Job Summary and Qualifications

Full time (Monday - Friday, day shift), no nights, no on-call, no weekends, no holidays 

 DUTIES INCLUDE BUT NOT LIMITED TO:

 • Coordinating the provision of patient care services related to such aspects as psychosocial and needs assessment, crisis intervention, and emotional support 

• Research and advocate for appropriate public assistance resources for clients • Referral to community resources, assistance with socio-economic problems 

• Communicate with client’s care team

 • Provide crisis intervention (referral to DCF, provider, etc as needed) 

• Coordinate treatment planning with specialists and outpatient providers as needed

 • Evaluate current patients for appropriate social work services 

• Judge relative costs/benefits of potential social service and health care actions and choose the most appropriate one 

EDUCATION

• Master's Degree in social work (required) 

EXPERIENCE:

• Experience working with clients that are affected by issues including neglect, abuse, domestic violence, substance abuse 

• Minimum of 2 years prior experience 

CERTIFICATION/LICENSE: • State licensure or certification commensurate with degree  (LCSW)
