Summary: This position supports the Organization in the following manner:Provides counseling services for mental illness and substance use disorder.Completes screening, brief intervention, and referral to treatmentProactive builds a patient panel through patient outreach and engaging primary care and other providers.Maintains medical recordsFull-time PositionReports to: Behavioral Health and Social Services Program ManagerSupervises: NoDress Requirement:Business CasualWork Schedule:Monday through Friday during standard business hoursTimes are subject to change due to business necessityExempt Job Duties, these are considered essential to the successful performance of this position:Assist the providers as needed by administering the highest quality patient care in a sensitive and respectful mannerAccurately documentation in appropriate medical recordEnsure providers' orders are properly followedTriage patients in office visits, walk-ins and via telephone to complete SBIRT or crisis counseling services.Perform the tasks of an LSW or LCSWFollows all policies, procedures and work rulesOther duties as necessary or assignedJob Qualifications (Experience, Knowledge, Skills and Abilities):Must be Licensed in the State of Ohio to provide mental health counseling services (LSW, LPCC, or equivalent; LISW preferred)Required experience: at least one-year CounselingRequired education: Bachelor's degree. Preferred: Master's degree.Must have experience in Mental Health counseling. Experience in Substance Use Disorder Counseling is strongly preferred.Experience with integrated behavioral health preferred.Multi-lingual (Spanish, Nepali, or Somali) and/or experience in working with interpreters/diverse patient populations preferred.Ability to successfully guide others to create an efficient, harmonious work environmentSupervision certification or supervision experience preferredKnowledge of federal, state and local laws and regulations pertaining to Social Work LicensesEMR/EHR experience requiredCompetency in working sensitively and 1respectfully with people of various cultures and social statusSkills to expertly deliver exceptional health care to the publicFacility Environment:Heart of Ohio Family Health Centers operates in multiple locations, in Columbus, OH. All facilities have a medical office environment with front-desk reception area, separate patient examination rooms, nursing stations, pharmacy stock room, business offices, hallways and private toilet facilities. All facilities are on the main ground floor and ADA compliant.This position's primary work area is the clinical portion of the facility; ie, patient examination rooms, hallways, and other clinically assigned areas.The clinical area is:kept at a normal working temperaturesanitized dailymaintains standard environment furniture with adjustable chairsmaintains standard office equipment; ie, computer, copier, fax machine, etc. at a normal working height
