What You Will Do
- Manage a social work caseload, supporting patients and families experiencing social, emotional, or environmental challenges that impact their health
- Collaborate closely with physicians, nursing staff, hospital personnel, and community agencies to ensure coordinated, high‑quality care
- Participate in case conferences to assess social needs, family dynamics, strengths, and barriers to care
- Advocate for patients by connecting them to community resources and supporting transitions across care settings
- Help patients and families understand and follow medical recommendations to support optimal health outcomes
- Maintain accurate clinical documentation and provide timely progress updates to physicians and care teams
- Serve as a resource and educator to clinical staff on how emotional and social factors affect physical health
- Build and maintain relationships with external clinicians and community partners to meet patient needs
- Actively promote excellent customer service, patient safety, and organizational values
What You Will Need
- Master's degree in Social Work (MSW)
- Active and valid Indiana social work license
- Minimum of one year of social work experience in a medical, healthcare, or mental health setting
- Strong clinical judgment, critical thinking, and problem‑solving skills
- Ability to communicate clearly and effectively with patients, families, clinicians, and community partners
- Comfort working collaboratively in a multidisciplinary healthcare environment
- Basic competency with electronic medical records and computer applications
