Agawam North Rehab & Nursing is seeking a licensed social worker (MSW or BSW) to lead care coordination, discharge planning, and psychosocial support services across our facility. This role offers the ability to operate with autonomy, leadership visibility, and direct impact on resident outcomes.


Compensation

  • MSW (Licensed): $83,000 - $93,000/year
  • BSW (Licensed): $65,000 - $75,000/year

Core Focus of the Role

Care Transitions & Discharge Outcomes

  • Evaluate incoming referrals and support admission decisions
  • Coordinate and execute safe, timely discharges with appropriate services in place
  • Partner with families and external providers to ensure continuity of care

Resident Support & Clinical Assessments

  • Complete psychosocial assessments including BIMS, PHQ-9, and related evaluations
  • Provide ongoing support to residents and families navigating care changes
  • Assist with advance directives and long-term planning needs

Care Planning & Collaboration

  • Participate in care planning discussions and help align resident goals with services provided
  • Work closely with nursing, rehabilitation, and leadership to ensure coordinated care delivery

Operational Oversight & Compliance

  • Maintain accurate and timely documentation within PointClickCare
  • Support regulatory readiness and adherence to state and federal requirements
  • Contribute to process improvements within social services and care coordination workflows

Qualifications

  • Active Social Work license (LCSW, LICSW, or LSW)
  • MSW or BSW accepted (compensation aligned accordingly)
  • Experience in skilled nursing, rehabilitation, or long-term care required
  • Background in admissions, discharge planning, or care coordination
  • Knowledge of Medicaid processes
  • Strong communication skills and ability to work independently
