Licensed Practical/Vocational Nurse (LPN/LVN)

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide high-quality nursing care under the supervision of Registered Nurses (RNs) and physicians. The LPN will assist with patient assessments, medication administration, treatments, documentation, and daily patient care while ensuring patient safety and comfort.

• Provide direct patient care in accordance with the care plan.
• Administer medications and treatments as prescribed by the physician.
• Monitor and document patients' vital signs, symptoms, and overall condition.
• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility.
• Perform wound care, dressing changes, catheter care, and other nursing procedures within scope of practice.
• Accurately document patient care, observations, and interventions in the electronic medical record (EMR).
• Communicate patient status and any changes to the RN, physician, and interdisciplinary healthcare team.
• Educate patients and their families on medications, treatments, and discharge instructions.
• Maintain a clean, safe, and therapeutic environment while following infection control protocols.
• Respond promptly to patient needs and medical emergencies.
• Ensure compliance with facility policies, state regulations, and nursing standards.
