QUALIFICATIONS

RESPONSIBILITIES

Under the supervision of a Registered Nurse or physician, the LPN provides direct patient care on an assigned unit, using sound judgment, professionalism, and adherence to Nursing Department standards. May assist in supervising support staff.

Key Responsibilities

  • Patient Care: Deliver safe, accurate nursing care; use diagnostic information to assess and evaluate patient needs.
  • Documentation: Complete timely, thorough assessments and updates, especially when patient conditions change.
  • Care Planning: Contribute to individualized care plans, adjust interventions as needed, and prioritize based on patient needs.
  • Collaboration: Work with RNs, physicians, Care Managers, and interdisciplinary teams to coordinate care and support discharge planning.
  • Patient Privacy: Maintain professionalism and respect during all evaluations and interactions.
