Description Benefits Position Summary and Job Duties

A licensed practical nurse (LPN) is a technical nurse who has been educated to perform nursing intervention under the direction of a registered nurse in compliance with the nursing service policies and procedures. This position has responsibility to deliver selected treatments/procedures/tests; medications and patient care as delegated and directed by the RN.

The LPN assists the RN to monitor the care provided by the nursing assistants, including personal care and restorative nursing programs. The LPN is assigned to a neighborhood with responsibility for 16 to 32 residents.

Responsibilities ----------------

  • Administer medications and performs treatments (40%)
  • Administer medications prescribed for routine administration including those given via oral, subQ, IM, nasal, eye, ear, vaginal, rectal, inhaler and nebulizer routes.
  • Observes the effects of medications, reports to the RN or MD any unusual or adverse reactions.
  • Assures compliance with the State and Federal guidelines related to medication pass.
  • Administers IV therapy and IV prepared piggyback medications and installs heparin and saline flushes.
  • Completes blood draws once trained.
  • Administers PRN medications as indicated, consulting with the RN as needed.
  • Performs treatments after skills training including:
  • Clean and sterile dressing changes
  • Tracheostomy care and changes
  • Skin tear protocol
  • Catheter care and changes
  • Colostomy care
  • Blood sugar checks
  • G/J tube care and feeding
  • Peritoneal dialysis
  • Oxygen administration
  • Suctioning and nebulizer use.
  • Performs EKGs and bladder scans and vital sign assessment using the Welch Allyn machine.

Data Collection and Record Keeping ---------------------------------

  • Collects and records data which will be incorporated into nursing assessments and participates in the overall implementation of the residents plan of care.
  • Prepares incident/injury reports, weekly summaries, vital signs and physical symptoms, response to analgesics and other medications.
  • Immediately reports resident health concerns to the RN.
  • Assists the RN to provide guidance and supervision to the nursing assistants.
  • Participates in the overall planning and implementation of the care plan, PNA sheets and restorative plan of care.
  • Documents medication administration, treatments, treatment progress notes, weekly summaries and restorative nursing notes, and data gathering related to the residents health status.

Resident Care -------------

  • Monitors and assists with resident dining.
  • Assures that the care needs of high-risk residents are followed.
  • Assists feeding residents at risk for swallowing concerns.

Leadership and Teamwork ----------------------

  • May function as the team leader on the unit.
  • May function as the team leader during code situations (Code Blue, Code Black, Code Orange and Red Alert).
  • Administers emergency care including CPR and use of the AED.

Job Requirements, Education, Training & Experience ------------------------------------------------

  • Licensed Practical Nurse with current Wisconsin Licensure in good standing.
  • Long-term care experience preferred.
  • Completion of the expanded role LPN skills preferred.
  • CPR Certification or evidence of continued attendance at CPR updates.
  • May need to stay past the normal shift until relieved in order to meet minimal staffing requirements.

  • Bereavement Leave (Policy 5.04):
  • In the event of a death, an employee may be excused from work without loss of pay for up to a maximum of thirty-two (32) hours annually for the purpose of attending a person's wake, visitation, memorial service, funeral, or make necessary arrangements regarding the person's death, within a reasonable time after the occurrence.
  • Reasonable time after the occurrence is defined as within six (6) months from the date of death.
  • No more than (32) hours can be used for the same decedent.
  • If additional time is required beyond the thirty-two (32) hours annually, an employee may request to use accumulated vacation, holiday, or comp-time.
  • Sick leave can be used for a parent, spouse, or child only.
  • Bereavement leave cannot be accrued from one year to the next.
  • Bereavement leave can be used in increments of quarter hours.
  • Employee Assistance Program (EAP):
  • The County provides an Employee Assistance Program (EAP) for employees to use for all kinds of personal and professional assistance.
  • In the case of counseling, professional counselors are available 24 hours a day, 7 days a week.
  • An employee can establish a relationship with a counselor over the phone or set up an in-person appointment with a counselor in the area.
  • As of March 1, 2022, the EAP program will pay for up to six visits.
