Schedule: Full-Time Night Shift; (7pm - 7am). 3 days one week, 4 days the next, with an every other weekend rotation required

The Hoy Center at Westminster-Canterbury on Chesapeake Bay takes a holistic approach to skilled nursing care and aging. The Licensed Practical Nurse (LPN) provides nursing and clinical intervention and medication administration for assigned residents on a nursing unit and insures smooth operations of the unit for a designate shift. Administers medications for the unit(s) assigned in compliance with state regulations. What Youll Do In This Role:

  • Perform blood glucose monitoring, inhalation treatments, and other physical prescribed medical treatments/diagnostic test.
  • Maintain medication and residents records, responsible for electronic input of residents data in EHR system.
  • Transmit requests including scans and/or faxes medication refill requests per policy.
  • Communicate and coordinate care with residents and their families.
  • Document and lead in the documentation of resident care.
  • Inspect the environment, identify environmental problems and initiate requests for correction as needed.
  • Identify, report and document incidents and accidents; participate in investigation as necessary.
  • Review, transcribe and implement physician orders.
  • Communicate with residents physicians as directed.
  • Implement, practice and assure staff adheres to infection control measures.
  • Lead CNAs (Certified Nursing Assistant).
  • Participate in continuing education programs to maintain competency.
  • Support and participate in community and departmental quality assurance programs.
  • Ensure that unit practices are consistent with regulatory requirements.
  • Perform all other duties as assigned.

Physical Requirements:

  • Must be able to climb or descent 3-flights of stairs
  • Must be able to stand or walk for an extended period of time and push a wheeled medication cart through assigned unit

The Experience Youll Bring

Required

  • Active, unencumbered LPN license in the state of Virginia (or compact)
  • Current CPR certification
  • Must have at least one year of nursing experience, one of which is in a long-term care facility.
  • Flu-Vaccinated

Preferred

  • Experience with Microsoft and Google Applications.
