Overview
We are seeking a full-time Nurse (LPN or RN) to join our team. This position provides essential nursing services within the Madison and Granite City Housing Programs, supporting individuals receiving behavioral health, substance use, and primary care services. Level placement (LPN or RN ) will be determined based on licensure, experience, and qualifications. This role plays a critical part in delivering integrated, trauma-informed care and ensuring continuity of services across providers and settings.

Responsibilities
- Complete and maintain timely, accurate documentation including medical records, progress notes, treatment plans, and required funder documentation
- Administer, monitor, and educate clients on medications in accordance with treatment plans
- Assess clients for acute and ongoing physical health needs, providing first aid and coordinating care as needed
- Review medical records for completeness and compliance, including required signatures and documentation
- Coordinate care and act as liaison between prescribers, clients, pharmacies, labs, primary care providers, and other healthcare professionals
- Respond promptly to client and family inquiries regarding medications, refills, and health concerns
- Administer and document TB skin tests
- Provide holistic health education and promote wellness using nurse practice guidelines
- Support recovery-oriented, trauma-informed care emphasizing empowerment, respect, and self-directed recovery
- Participate in team meetings, staffings, and program development activities
- Maintain confidentiality and adhere to all organizational and regulatory standards
Qualifications
LPN Level:
- High school diploma or equivalent
- Licensed Practical Nurse (LPN) in the State of Illinois with less than 5 years of experience
- Basic computer skills, including Microsoft Word, Outlook, e-prescribing systems, and electronic medical records (EMR)
RN Level:
- High school diploma or equivalent
- Registered Nurse (RN) licensed in the State of Illinois
- Advanced proficiency in documentation systems and clinical coordination

The anticipated starting pay for new hires for this position is between $19.85 - $40.84 per hour.
