Base Pay: $25.00-$29.55 per hour (based on experience)
Shift: 3:00 PM-11:00 PM

We're seeking a compassionate and dependable Licensed Practical Nurse (LPN) Charge Nurse to provide exceptional resident care while leading the nursing team during the 3:00 PM-11:00 PM evening shift. Licensed Practical Nurse (LPN) – Charge Nurse Responsibilities
- Provide direct nursing care to residents in a skilled nursing, rehabilitation, and long-term care setting.
- Assess, monitor, and document changes in resident conditions.
- Administer medications, treatments, and nursing interventions according to physician orders.
- Lead and supervise Certified Nursing Assistants (CNAs) while supporting quality resident care.
- Coordinate resident care with Registered Nurses (RNs), physicians, therapists, and the interdisciplinary healthcare team.
- Respond promptly to changes in resident condition and communicate clinical concerns appropriately.
- Maintain accurate electronic nursing documentation in compliance with state and federal regulations.
- Promote infection prevention, resident safety, and quality improvement initiatives.
- Foster a positive, supportive, and team-oriented work environment.
Qualifications
- Current Licensed Practical Nurse (LPN) license in good standing.
- Current CPR/BLS certification.
- Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric nursing is preferred but not required.
- Previous Charge Nurse or leadership experience is preferred but not required.
- Strong clinical assessment, communication, leadership, and organizational skills.
