Community-Based Assessments (80%) + Be the face of Medicaid in the community by conducting in-home level of care assessments to determine eligibility for: Personal Care Services (PCS) and Aged & Disabled (A&D) Waiver programs. You'll travel to participants' homes, explain available services, and complete comprehensive evaluations to help individuals access Home and Community-Based Services (HCBS)-allowing them to remain in their homes rather than entering institutional care. Your assessments directly support participants with essential daily needs like bathing, dressing, and meal preparation. Stakeholder Support & Customer Service (15%) + Serve as a trusted resource for participants, families, and providers by: Answering questions about HCBS and supplemental services, assisting with provider-related concerns, and clarifying service authorizations and eligibility. Your guidance ensures participants receive the right care at the right time. Administrative & Team Collaboration (5%) + Support the team and maintain program integrity through: Documentation and correspondence, Participation in staff meetings, and General office duties that keep operations running smoothly Quality & Compliance Responsibilities + In addition to your core duties, you'll help uphold the highest standards of care by: Conducting regulatory compliance reviews through client sampling, assessing clients' health, emotional, and rehabilitative needs, observing care delivery and conducting interviews to ensure services meet state and federal standards, applying critical thinking and clinical judgment to evaluate care quality, and Writing clear, concise, and legally sound reports.
