    What You'll Do

    • Provide skilled hospice nursing including medication administration, symptom management, wound care, and ongoing patient monitoring.
    • Observe and report changes in patient condition while collaborating with the RN Case Manager.
    • Educate patients and caregivers on medications, comfort measures, and hospice services.
    • Build trusting relationships through regular visits in homes, assisted living communities, memory care, and skilled nursing facilities.
    • Complete accurate EMR documentation and participate in scheduled weekend, holiday, and on-call rotations.

    What You'll Bring

    • Active Minnesota Licensed Practical Nurse (LPN) license.
    • Experience in hospice, home health, long-term care, skilled nursing, oncology, or palliative care required.
    • Strong assessment, communication, and organizational skills.
    • Ability to work independently while collaborating closely with the interdisciplinary hospice team.
    • A passion for providing thoughtful, relationship-centered care.
