Location: Zachary, LA 70791 

Contract Type: Contract 

Shift: Monday–Friday, 8:00 AM–4:30 PM 

Mileage Reimbursement: Yes (from first patient to last patient) at .725 per mile

Rate: $30.00 per hour

About the Role

The LPN will provide direct patient care in a home health setting, supporting daily clinical needs and ensuring high‑quality, compliant care. This role requires strong assessment skills, accurate documentation, and the ability to work independently in the field.

Key Responsibilities

• Deliver patient care in accordance with agency policies and clinical standards

• Conduct assessments and document findings accurately in the EMR

• Provide wound care, medication support, and patient education

• Communicate effectively with the care team to support patient outcomes

• Maintain compliance with infection control and regulatory requirements

Required Qualifications

• Active LPN license (state‑specific or compact)

• Minimum of an associate degree

• Experience in home health strongly preferred

• Strong assessment and documentation skills

• Reliable transportation and ability to work independently

• Professional certification required

• Occupational health screenings required

Preferred Skills

• Wound care experience

• Familiarity with HCHB (HomeCare HomeBase)

• Strong clinical judgment and communication skills

Assignment Details

• Standard 40‑hour workweek

• No on‑call or weekend requirement

• Coverage area: Zachary, LA and surrounding patient locations

• Contract role (not contract‑to‑hire)
