The incumbent serves as a member of the health care team with selected patients and provides care to them on a regular and recurring basis. The LPN 6 completes assigned duties and responsibilities which require a broad work experience. Recurring duties are carried out independently. The LPN recognizes the need for and seeks guidance when necessary to ensure quality patient care. In organizing the delivery of care, the LPN recognizes and considers emotional, cultural, spiritual, and age related factors. The LPN actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice.
The Veterans Health Administration (VHA) expands the provision of patient care to various institutional and non-institutional settings, such as the Department of Veterans Affairs Medical Centers (VAMCs), Community-Based Outpatient Clinics (CBOCs), and other affiliated organizations by utilizing innovative telecommunication technologies to enhance care coordination, access to care, and to routinely and proactively manage diseases to prevent healthcare crises. These programs may include, but are not limited to, Clinical Video Telehealth (CVT), Home Telehealth (HT), and Store and Forward (SF).
The Telehealth position is one that serves in a generalist role to support Telehealth clinical programs from the patient location. The incumbent provides a wide range of clinical and technical services to the veteran that demonstrates complex technical judgment under general supervision of a registered nurse or MD. The complexity of duties includes serving as the site imager for Telehealth store and forward applications, facilitator for real time Clinical Video Telehealth (CVT) encounters, including patient education activities, Primary Care as well as specialty care remote visits, and providing equipment for the e Telehealth (HT) program. Additional provides technical and scheduling support, and other program support duties, as assigned.

GS-03 None beyond the basic requirements. LPNs at this grade level are expected to perform all duties within the scope of their license (e.g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved.
GS-04 Six months of qualifying experience as an LPN or LVN; or (b) Graduation from an approved school (which may be waived as provided in paragraph 4 of Section C) and one year of experience that involved nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; or (c) Graduation from an approved school of at least 24 months duration.
GS-05 Completion of at least 1 year of qualifying experience at the GS-4 level or equivalent.

Physical Requirements: The incumbent will be required to move throughout the clinic to interrelate with staff, patients, physicians, other medical center providers, and administrative staff. This position involves considerable amount of standing, walking, stooping, bending, reaching, lifting, and turning. It requires some repositioning patients, assisting with transfers from wheelchairs/stretchers and transporting them to other departments or facilities. Mechanical devices are available and are required to be used for heavy or difficult to lift clients. The work may involve pushing or lifting more than 50 pounds and going up and down flights of stairs. The incumbent is expected to use proper [preventive ergonomic techniques to perform all duties. The position requires visual and auditory acuity at a level to provide safe and effective nursing care. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The position also requires ability to work on computers for extended periods of time. The incumbent must be able to use the computer without limitations. The incumbent may be exposed to infected patients and contaminated materials any may be required to don protective clothing during episodes of care.
