THE OPPORTUNITY
We are seeking a dedicated and compassionate Licensed Practical Nurse to join our Healthcare Services team as an LPN Case Coordinator. In this vital role, you will make a meaningful difference in the lives of individuals with developmental disabilities by managing their healthcare needs, coordinating medical services, and ensuring the highest standards of care and safety.
As an LPN Case Coordinator, you will oversee a caseload of residential homes, working directly with individuals, families, healthcare providers, and support staff to deliver comprehensive, person-centered healthcare coordination. This position offers the opportunity to utilize your clinical skills while building meaningful relationships and advocating for those who depend on your expertise.
WHAT YOU'LL DO

Required Qualifications:
â Graduate of an accredited school of nursing
â Current State of Ohio LPN license in good standing
â Valid Ohio Driver's License with acceptable driving record
â Ability to lift up to 50 pounds and perform physical tasks
