Schedule: Variable Day & Evening Shifts | 8-hour shifts
Guaranteed Hours: 40 hours per week
Compensation:
- Local: From $35/hour
- Travel: From $1,600/week
Position Summary
We are seeking a reliable and experienced Licensed Practical Nurse (LPN) to provide high-quality nursing care in a skilled nursing and rehabilitation setting.
This role involves delivering compassionate, resident-centered care, supporting clinical operations, and collaborating with interdisciplinary teams to ensure optimal patient outcomes. The ideal candidate is flexible and comfortable working day and evening shifts.
Key Responsibilities
Patient Care & Treatment
- Administer medications and treatments as prescribed
- Provide direct nursing care while ensuring resident safety and comfort
- Monitor resident conditions and report changes to the care team
Clinical Support & Documentation
- Assist with:
- Resident assessments
- Care planning
- Clinical documentation
- Maintain accurate and timely patient records
Team Collaboration
- Work closely with:
- Certified Nursing Assistants (CNAs)
- Nursing staff
- Interdisciplinary care teams
- Support coordinated and efficient care delivery
Requirements
- Active Licensed Practical Nurse (LPN) license (required)
- Experience in:
- Long-Term Care (LTC) or
- Skilled Nursing Facility (SNF) (preferred)
- Ability to work:
- Rotating day and evening shifts
- Every other weekend
- Flexibility to work 5 shifts per week as needed
