Licensed Practical Nurse - Licensed Vocational Nurse Position Summary

As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice. Reports to the Director or Manager of Department.

Essential Functions

• Contributes to the assessment of patients by collecting data for analysis by an RN. • Performs plan of care interventions, including specimen collection and wound care; may administer medication. • Documents patient care provided. • Assists RN with development of plan of care. • Assists patients with daily living activities including personal hygiene, elimination, nutrition, and ambulation. • Monitors, documents, and communicates patient status and responses to interventions with RN, physician, patient, and family. • Advocates for the physical and emotional well-being of the patient. • Participates in performance improvement and continuing education. • Provides quality patient care and works flexible shifts/units as needed. • Maintains regular and reliable attendance. • Performs other duties as assigned.

Knowledge, Skills & Abilities

Education: • Graduate of accredited LPN program.

License: • Current LPN license in state of practice. • BCLS required. • Handle with Care preferred.
