As a Home Health LPN, you’ll provide one-on-one patient care, support recovery, educate patients and families, and work alongside a collaborative clinical team focused on positive patient outcomes.

What You’ll Do
Provide skilled nursing care in patients’ homes under physician orders
Follow care plans and goals created by the RN Case Manager
Monitor patient conditions and report changes
Administer medications and treatments as prescribed
Educate patients and families on disease management and preventative care
Help reduce re-hospitalizations through patient education and proactive care
Complete clinical documentation in accordance with agency and Medicare guidelines
Participate in on-call, case conferences, trainings, and required meetings
Qualifications
Current Licensed Practical Nurse license in the state of employment
Minimum of 1 year of LPN experience in an acute care setting required
2+ years preferred
Home health experience preferred
Valid driver’s license and active auto insurance
Strong communication, time management, and documentation skills
Compassionate, dependable, and patient-focused
