Duties Description Are you ready to start your Social Work career with New York State? Hutchings Psychiatric Center is recruiting for a Licensed Master Social Worker to provide professional social work services to individuals with mental illness. Candidates from outside or within State Service can apply.
The successful candidate will provide clinical and support services in the community to successfully maintain each person in his/her home or community. The multidisciplinary staff comprised of professionals and paraprofessionals will offer a network of community-based services that support, promote and maintain health, hope, resilience and recovery. The successful candidate will provide brief therapeutic support using evidence-based practices to help teach/support symptom management skills, strengthen coping skills, improve medication management skills and reduce crisis situations, develop and implement groups in the community to help support and improve individuals functioning in the community. Will also provide case management services in the areas of linking-individuals to community-based services and coordinating services with other providers, family involvement, advocacy such as accompanying person to community appointments and community integration - achieving or maintaining independent living and functioning in the community. Additionally, this candidate will work collaboratively with clinic staff to perform wellness checks on individuals lost to contact or difficult to engage. Required documentation includes, but is not limited to, daily contact notes, service recording, GOMPS and individual service plans and reviews, progress notes, referral forms, and correspondence with other agencies.
Duties include:
Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program if they are licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW).
OR
Candidates within State Service can be considered transfer to a competitive class position if they have one year of permanent competitive or 55-b/55-c service in a title eligible for transfer under Section 70.1 of the Civil Service Law.

This position is for the Mobile Integration Team - State Operated teams providing an array of mobile services and supports for adults, including on site crisis assessment, family support and respite.
Must have a valid drivers License and be able to drive a state vehicle.
NOTE:
If you fail to maintain your license and/or registration once you are appointed, action will be taken to remove you from the position.

If appointed you may be required to become an enrolled Medicare provider; obtain and provide to your employer a National Provider Identifier (NPI) number issued by the National Plan and Provider Enumeration System (NPPES); and otherwise actively participate to the degree necessary to allow for your services to be billed through Medicare and Medicaid. If you are appointed and you lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.
